Enrollment Options

CBCA offers families three options under the same price structure.  Our record keeping program offers members full access to record keeping, fine arts, and athletics, and provides students with a transcript/diploma upon completion of high school.  Our associate program offers access to all fine arts/athletics aspects and does not include record keeping.  Our remote records program is available to families who desire record keeping only.

FULL Program

Associate Program*

  • Access to CBCA fine arts and athletics

  • Register records with county or other provider of choice
    *CBCA does not maintain records or provide transcript/diploma.
    *Associate students may participate in our commencement exercises.

Remote Program (specific conditions apply)

  • CBCA maintains academic transcript

  • Parents select curriculum

  • Parents submit quarterly grades/lesson plans/attendance records

  • Annual academic assessment required

  • High school diploma/transcript provided

How to Enroll

Enrollment steps are:

  1. Submit your Application Form (first part of New Student Packet) in one of the following ways:

    a. Scan and E-mail it to schooloffice@calvaryonline.com

    b. Drop it off at the school Tuesday-Friday 8-5 (1945 N. Florida Ave. Lakeland)

    c. Bring to the interview

  2. To schedule an interview, please e-mail Carol Bates at: carol@calvaryonline.com or call the school office at (863) 683-6781. Acceptance to CBCA will be verified after the interview.

  3. The remaining pages of the New Student Packet (Registration Form) will be required upon acceptance. Feel free to fill out this form now or wait until after the interview and acceptance. Once completed, submit by e-mailing it to schooloffice@calvaryonline.com or dropping it off at the school.

  4. Provide required documents:

    • School entry health exam (Florida Form DH 3040)

    • Immunization record (Florida Form 680 or Exemption Form DH681)

    • Copy of birth certificate

    • Transcript of grades from previous school year *

    • Assessment results from previous school year *

* for students enrolling in the record keeping program
